As requested, I have obtained the following quote for professional liability 
insurance for the officers and directors.
Limits                Annual Premium
$500,000                    $856
$1,000,000                $1.070
$2,000,000                $1,605
$3,000,000                $2,070
$4,000,000                $2,570
$5,000,000                $3,070

This is on a claims made basis, which means the policy must be in effect at the 
time a claim is made. It differs from the occurrence form as that form would 
respond if the coverage were no longer in effect at the time the claim is made, 
but there was coverage on the date of the occurrence.
