Comments as to form:

AGA Group Discount Policy application:

"including participation in the AGA e-mail members list,
discounts in the AGA
online bookstore, and AGA convention registration"
should not have the comma before "and AGA". With the comma,
it means they get free registrations.

I think you can leave off the -5 memberships line since
there is no discount.

AGA Group Discount & Premium Club Application:

"New issues of The Aquatic Gardener will be mailed to a
single physical address. "

Change to " All copies of The Aquatic Gardener will be
mailed to a single physical address." 

"I have read these rules and being duly authorized by my
club to do so, for and on behalf of my club,
_________________________________________________________ I
agree to abide by the rules."

Change to: "I ____________________________ on ______
               <name of person signing>       <date>
have read these rules and being duly authorized by my club
__________________________________
<name of club or oranization>

to do so, for and on behalf of my club, by my signature 
agree to abide by the rules."

Replace "Signature of primary contact person" with "Print
name of primary contact person
