I don’t know the “Official” form number or name, but you are right about its purpose. The form comes from the Secretary of State and is your affirmation that GSAS is still a non-profit corporation. Being a corporation, you must restate the “purpose” of the organization (mission statement is perfect) and then provide the names and addresses of the current officers.
