That sounds exactly like what happened with my tank of death! Water chemistry
was spot on, and the last ten or so fish that died never appeared to have had
ich.

After adding the coppersafe, salt, and continuing the rid-ich last night as
directed, I came home to half of my tank dead this afternoon. All the clown
loaches and everything over 4 inches long died. The fish remaining were
stressed, dark-colored, and gasping at the surface. Basic water chemistry
checked out OK, so I -still- don't know what went on.
<sarcasm> There certainly doesn't appear to be any ich left in the tank now.
Everything that had it died, plus five gorgeous old fish that didn't have it
at
all. </>
