Sarah I've seen alot of Cacatuoides in pet shops that didn't have anywhere near the color of Altispinosa's have when they reach 3" in size. I really love that fish. The deep gold mustard colored body with blood red fins is extremely attractive. Now as for the 3rd fish in a 20 long. I wouldn't attempt to add another male to try and have 2 pairs breeding in the same tank. I fear all hell would break out if that was tried. Maybe in a 55 where there's more space. You could try to build a natural bridge/divider in the middle of the tank with rocks and driftwood that would sort of act as a natural divider for the tank. Then maybe the male might swim over and mate with the other female. Personally I would just remove her to another tank and get another male for her. It's always nice to have 2 pairs of such a nice looking fish. The second pair could become trade bait for someone looking for a pair and that has some dwarf you might be looking for.
