Head injury?

If my at least 3 year old Opaline Gourami
http://uniquaria.com/photogallery/gourami/mgmopaline.jpg suffered a head
injury, what would it
look like?

He currently has a purplish looking area on the top of his head - looks like a
bruise [see
attached photo]. He has been hanging around at the surface of the water with
his head right at
the surface and his body hanging down diagonally to almost vertically below the
surface [he
used to mostly stay close to the bottom in a driftwood "cave" he's always been
timid]. The few
times he has left the surface and gone to the mid-water, he looks very clumsy
and eventually
returns to the top. His body does not look bloated.

He is still eating well and interacting with the female when she comes near the
surface.

I just don't know if there is anything I can do for him? His skin doesn't look
broken, just
discolored in that one area. I hate to treat with medicine if I don't know why
[plus I don't
have any here anyway]. Plus, if it is a head injury, medicine is not going to
help. But he's
not acting normally.

Any suggestions?
