There may be some physical injuries involved but there may also be some internal problems caused by a chronic vitamin deficiency or some spontaneous malfunction (he is three years old). Patches of abnormally dark color on a gourami can be caused by either and are common in those fish. Sometimes it is an injury along the lateral line that affects the nerves. I would lean toward the vitamin deficiency idea because of the tail-drooping you described. These guys have the swim bladder in the back of the body to balance against the bubble they hold in their mouth, so he probably is suffering from a swim bladder malfunction, the exact cause of which usually remains a mystery. If you want to try something you might start with some foods that are rich in vitamins and think about adding some minerals (maybe in the form of marine salt). And double check the temperature and pH as well as doing a water change just on general principles.
