Yeah, food was the first thing that came to my mind. It could also be: Water temperature The nature of the water added to the tank at water-change time might be a problem (chlorine, chloramine, hardness, ph, etc.) Disease/parasites (already mentioned) There are probably other things I haven't thought of.
